Conditions:

1. Please fill out this RMA Request Form acurately and completely, and fax to Harmony along with the original Packing Lists & Invoices to obtain a Harmony RMA number.

2. Products returned to Harmony Multimedia without a Harmony RMA number indicated outside of boxes could result in shipment rejection.

3. Products returned to Harmony Multimedia must be consistent to the data on the Request Form. Otherwise, Harmony RMA department reserve the right to either a) return the shipment to the customer, b) correct and inform the customer about the mistake, or c) handle the RMA according to the data on the RMA Reguest Form.

4. Product warranties are void if profucts returned to Harmony Multimedia have physical damage.

5. DO NOT send back the packing accessory or boxes. Harmony Multimedia will replace and return the modem cards only.

6. Harmony Multimedia does not perform cross shipping services.

7. Please allow up to 15 working days for RMA processing.
